Our intake forms and waivers are located in the Vagaro Scheduling system.

 

Please be aware if you have used this system for other related businesses you may already have an account that can be used to book appointments at Mantra.

In order to book an appointment the health history and waiver must be filled out  as well as a credit card put on file. If you have a gift card, you will still need to put a credit card on file, as we do charge for less than 24 hour notice cancelations and no- show situations.
